Please click on the Membership navigation button on our home page and print the membership application that is found there. You do not need to join our organization to make a contribution, but we will need the information called for on the form and you will need to send your contribution to the address that appears on the form.

As a Compassion & Choices donor and champion of choice in dying, you:
- Fund our End-of-Life Consultation Program that counsels more than 100 new clients facing end-of-life choices each month;
- Support our drive to enact and protect aid-in-dying laws, including our landmark U.S. Supreme Court victory in Gonzales v. Oregon;
- And back our mission to educate the public on the critical need to complete an advance directive and talk to family and health care providers about end-of-life care wishes.
