Compassion & Choices is a membership organization. When you join, you declare your support for choice and improved care for terminally ill patients at the end of life. Even more, you help us to advocate for our clients and their families, and provide much needed outreach to the public and to lawmakers.
Membership has additional benefits:
GENERAL MEMBERSHIP
Cost: Individual - $45 a year Couple/Dual - $60 a year
Benefits:
• Access to our Client Support Program, with information and counsel on issues related to the end of life.
• Membership in your state or local chapter of C&C, where available.
• State-specific advance directive forms and free assistance in creating and utilizing your advance directive.
• Subscription to Compassion & Choices magazine, which is issued quarterly.
• Discounts on Compassion & Choices products, including books, pamphlets and videos.
BENEFACTOR MEMBERSHIP
Cost: $100 a year
Same benefits as General Membership plus:
• Wallet-sized CD of your advance directive.
• Listing in our annual report.
LIFE MEMBERSHIP
Cost: Individual - $450 Couple/Dual - $600
Same benefits as General Membership plus:
• Wallet-sized CD of your advance directive
• Life member recognition certificate
• Listing in new Life Members in our annual report

If you would like to join our organization, please print our membership form, fill it out and mail it to the address that appears on the form. Membership in the national organization automatically includes membership in the Wisconsin chapter for Wisconsin residents.
