What is the End-of-Life Consultation Program? Compassion & Choices' End-of-Life Consultation Program serves individuals who are terminally ill, their families and others who just want to plan ahead. These confidential services include:
1. Information and support to anyone who contacts us. 2. Local referrals to pain specialists, hospice programs, social service agencies, disease-specific support groups and others. 3. Assistance in completing living wills and powers of attorney for health care, and advice on talking to families, friends and health care providers about health care wishes. 4. Advocacy for people in nursing homes or others who are receiving inadequate care in other settings.
The End-of-Life Consultation Program was previously called Compassion & Choices' Client Support Program. It is a continuation of End-of-Life Choice's Caring Friends Program and Compassion in Dying's Case Management Program.
How do I contact the End-of-Life Consultation Program to ask for information or assistance? If you would like more information about the program or would like to apply for services, please call our toll free telephone number: 800-247-7421. If the caller lives in an area where the program has trained volunteers, he or she may be offered a volunteer who can work with the person locally. However, all calls need to come through the toll free 800 number before a local volunteer can be assigned. If there is no volunteer in the immediate area, the national organization's counselor will work with the caller by phone.
Do the services of the End-of-Life Consultation Program cost anything? No. All services included in the End-of-Life Consultation Program are provided free of charge.
What if someone who does not have a terminal illness contacts Compassion & Choices' End-of-Life Consultation Program? No one who contacts the End-of-Life Consultation Program is turned away. The program will provide information to qualified individuals who are not in the terminal stage of disease, as well as those who are terminally ill. In addition, if the program is unable to serve someone's needs, that person will be appropriately referred, and the End-of-Life Consultation Program with work with them until the referral is in place.
If I am terminally ill and wish to hasten my death, will the End-of-Life Consultation Program provide me with the means to do so? Compassion & Choices does not provide the means to hasten death, nor do we administer the means to hasten death. We offer information and support to qualified clients who wish to hasten their deaths.
